The Emergency Ultrasound Fellowship Accreditation Council (EUFAC) emailed the faculty and matriculating fellows of EUFAC accredited fellowships with an annual questionnaire today.

Please note, that there was an error in the system and the emails contained incorrect academic year and deadline dates. The links within the email can still be utilized to complete the questionnaires, regardless of the inaccurate dates listed in the email.

Please make your faculty and fellows aware to check their inboxes for these emails, as completion of these questionnaires is a required component of continuing EUFAC accreditation.

The subject line for the emails includes “EUFAC Questionnaire Action Required”.

Deadline for response submissions to the questionnaires for both matriculating fellows and faculty is:
